1. Please select a date and time slot (final program times will be confirmed later).

2. We can accommodate up to 5 sessions of the same program per day.

3. A separate registration form must be completed for each grade level.

4. Prior to the program, we will need a schedule for the day, including:

• Teacher names

• Class times

• Number of students
